Architectural Control Committee (ACC) Form

As a homeowner, please complete this Submission Form before making any exterior changes to your home, such as changing house color; building a fence, deck, shed, playhouse, etc.; adding on to the house; adding a satellite dish; changing landscaping; or any other change/modification in conjunction with the Covenants, Conditions, and Restrictions (CC&Rs).

Referenced sections of the CC&Rs include but are not limited to; 3.1, 3.2.1, 3.2.3, 3.5.9, 3.5.18.

You may email your scanned, completed ACC form to info@vismanagement.com. Alternatively, print, fill out and return to: China Falls HOA c/o VIS Group, 8617 Martin Way E. Lacey, WA 98516. The Board will review the request and approve or deny no later than 45 days after request is made (sooner, if possible).
